As with all modelling studies, a number of limitations of the present study are worth noting. First, we have assumed standardized management strategies for patients presenting with heartburn symptoms based on survey responses from 55 family physicians and 48 gastroenterologists. Although physicians who participated in the survey were randomly selected from across Ontario, it is unclear how generalizable are their responses. It is likely that there are geographic differences in practice patterns, in waiting times for specialist referral, and in the timely availability of diagnostic tests and procedures such as endoscopy. Second, we used moderate-to-severe heartburn as our primary measure of GERD symptoms. This was primarily because of differences across studies in how GERD symptoms are measured and which symptoms are included in the analysis. Since heartburn is the predominant GERD symptom, most studies include a separate reporting of heartburn symptoms. It is uncertain how the results of this study might change if a different or more inclusive symptom definition was used. Third, all studies in our review that reported symptomatic heartburn recurrence were conducted in patients with erosive esophagitis. Therefore our analyses of ENRD and uninvestigated populations did not include population specific recurrence rates. Fourth, the 1-year time horizon chosen for the study may be too short to capture long-term complications such as Barrett's esophagus or esophageal stricture. Given the lack of long-term follow-up studies, we did not feel it was appropriate to extrapolate the model much beyond 1-year. Finally, this study uses inputs i.e. costs ; which are specific to the province of Ontario. Price weights and surveys of practice patterns from other geographic areas would be needed in order to fully explore the potential impact of regional variations in cost and practice pattern and rapamune.

Homocysteine is a thiol-containing amino acid produced intracellularly by demethylation of methionine [1, 2, 3, 4]. It is a key metabolite in amino acid synthesis. During the process of methylation, S-adenosylmethionine Ado Met ; , derived from methionine, is converted to S-adenosylhomocystine. This product is quickly hydrolyzed to form homocysteine and adenosine [2]. Homocysteine can undergo 1 or 3 reactions depending on the status of the organism. If cysteine levels are inadequate, homocysteine utilizes the coenzyme pyridoxal phosphate vitamin B6 ; to condense with serine, forming an intermediate product - cystathionine. Subsequent reactions with cystathionine lead to the formation of cysteine. When methionine levels are low, homocysteine is re-methylated in a reaction involving the coenzyme N5-methyltetrahydrofolate or betaine. Finally, when the supply of both amino acids is adequate, homocysteine is cleaved by the enzyme homocysteine desulfhydrase cystathionase ; to form a-ketobutyrate and ammonia. Thus, the physiological role of homocysteine is to assist in maintenance of sulfur-amino acid homeostasis [2]. Severely elevated concentrations of homocysteine in blood are observed in subjects with genetic defects. These include deficiencies of the trans-sulfuration enzyme cystathionine -synthase gene symbol: CBS ; , the disturbances of the remethylation enzymes 5, 10-methylenetetrahydro folate reductase gene symbol: MTHFR ; , methionine synthase gene symbol: MTR ; , and the recently identified methionine synthase reductase gene symbol: MTR ; . Other factors that influence elevated homocysteine levels include deficiency of vitamin B6 and B12, chronic renal diseases and manifestations of aging in elderly subjects [3, 4, 5, 6, The adverse effects associated with hyperhomocysteinemia are extensive. It increases the risk of myocardial infarction, cardiovascular diseases CVD ; -related morbidity and mortality, peripheral vascular disease, atherosclerosis, coronary heart disease, and cerebrovascular disease [3, 5]. Raised homocysteine blood levels cause endothelial lesions, promote smooth muscle cell proliferation and the degradation of proteoglycans in the vascular wall. Homocysteine activates the clotting cascade, increases the production of thromboxane and LDL lipids peroxydation, thus inducing the formation of atherosclerotic plaques and thrombosis [5, 6, 9, 10, Its significance as a risk factor has been equated to that of hypercholesterolemia and smoking, two leading causes of cardiovascular disease worldwide. It also has been shown to produce a multiplicative effect with these and other risk factors such as hypertension. Homocystinuric patients, who have severe hyperhomocysteinemia, die prematurely of atherothrombotic disease [10]. Many but not all ; crosssectional and prospective studies indicate, on the average, that plasma homocysteine levels above 10 mol L are associated with, or predict the development of coronary, cerebral, and peripheral vascular disease [9, 10]. The risk conferred by hyperhomocysteinemia is graded and is independent of traditional risk factors, with an estimated odds ratio for ischemic heart disease of 1.4 for every 5 mol L increase in the plasma homocysteine concentration.

Despite these improvements, which may significantly improve patient morbidity and mortality, most individuals on HAART, even those with an undetectable plasma viral load, continue to have low-grade, ongoing HIV replication. It has been speculated that this continued HIV replication may be responsible for chronically elevated levels of CD8 + T cells or abnormal CD4 + : CD8 + T cell ratios observed in some patients on HAART, and data from a recent study from the National Institutes of Health NIH ; support this conclusion. In the study, published in The Journal of Infectious Diseases, the investigators at the NIH evaluated the relationship between CD4 + : CD8 + T cell count ratios and the frequency of CD4 + T cells carrying HIV-1 proviral DNA and of HIV-specific cytotoxic CD8 + T lymphocytes CTLs ; . The study enrolled 32 HIV-infected individuals who had been receiving HAART for 2.5 years and in whom plasma viral load was 50 copies mL. In these patients, the mean CD4 + and CD8 + T cell counts were 840 cells mm3 and 1110 cells mm3, respectively, and the CD4 + : CD8 + T cell ratio was 0.90. Results The investigators found a statistically significant inverse correlation between the frequency of CD4 + T cells carrying HIV-1 proviral DNA and the CD4 + : CD8 + T cell ratio in these individuals. No correlation was found between the frequency of HIV1-specific cytotoxic CD8 + T lymphocytes and the CD4 + : CD8 + T cell ratios in those individuals. The authors conclude, "These data suggest that persistent, low-level, ongoing viral replication, although not sufficient to maintain HIV-1-specific CTL responses, may explain, in part, why normalization of the CD4 + : CD8 + T cell ratio is not achieved in some infected individuals successfully treated with HAART.

The demand for strengthening and repair of civil infrastructures are escalating to ensure the safety of community against deterioration and extreme acts of Mother Nature that can be deadly. The utilization of Fiber Reinforced Polymer FRP ; , a relatively new material in civil works is favorable to combat this challenge. However it being a brittle material does not entitle it to be designed to its fullest strength prompting a large amount of strength to about 40% to be disregarded. Hence by combining several types of fibers to produce a ductile Hybrid Fiber Reinforced Polymer HFRP ; , the conservative measure in design of FRP can be neglected. By the means of understanding the fundamentals and mechanical properties of fibers and adhesives, a satisfactory hybrid system that meets the design requirements can be appropriately manufactured and applied. In this study, the hybrid system encompasses the employment of carbon fiber, glass fiber, jute fiber and steel plates. Extensive research was performed at laboratory to investigate the material's characteristics. Initially each individual fiber were experimented and analyzed methodically. In the latter stage, the properties of individual fiber was then incorporated to theoretically and mathematically predict an adequate and satisfactory hybrid system, and was affirmed experimentally based on the ASTM standards for composites. The result obtained validates that a hybrid system is a success as it is capable and reliable of delivering expectations, and in addition provides ductility and a much more economical approach that primarily is the scope of this research.

The critical task now facing APEC is how to consolidate and build on the success of the initial phase. In PECC's view this will require first and foremost a successful outcome in terms of `APEC's competition principles', the purpose of which will be to guide progress towards achievement of the OAA objective. On the assumption that these principles reflect comprehensiveness as a core element, they will need to connect not only with APEC's interest in regulatory reform, but also with a range of other policy areas relevant to the OAA competition and market efficiency objective. Building on the initial phase of competition policy development will also necessitate a review and re-specification of the IAP guidelines and of the collective action pertaining to principles. The purpose of a set of competition principles is to guide the development of a competition-driven policy framework for APEC economies in order to create maintain those market conditions that will enable and encourage an efficient competitive process to work. In PECC's view, comprehensiveness along with non discrimination, in the sense of competitive neutrality, transparency and accountability, should comprise the core elements of any competition principles adopted by APEC economies. These are compatible with the general principles on which the OAA is based but have special meaning in the context of a competition framework geared to welfare maximization as distinct from trade maximization. The principle of flexibility also finds expression in the PECC Principles in terms of the transition arrangements which reflect the region's diversity. The hallmark of CML is the Philadelphia chromosome that arises from a reciprocal translocation between chromosomes 9 and 22.1, 2 The molecular consequence of this translocation is a bcr-abl chimeric gene whose protein product bcr-abl ; exhibits enhanced tyrosine kinase activity. This constitutively activated enzyme activity is essential to the transforming function of bcr-abl and is a major factor in the pathophysiology of CML.3 The presence of bcr-abl in all patients with CML and the requirement of kinase activity for bcr-abl function make this an attractive target for a selective kinase inhibitor. In the 1990s, Druker and colleagues 4 discovered a molecule, imatinib mesylate, that inhibited bcr-abl kinase activity.4, 5 This novel inhibitor was shown to be highly effective in blocking abl tyrosine kinase activity by binding and inactivating the adenosine triphosphatebinding pocket of abl6 in the leukemic cells in CML. It also inhibited the stem cell factor receptor c-kit CD117 ; 7 and the plateletderived growth factor receptor8 but had little effect on other tyrosine kinases.
